Systematic review on health care professionals' competencies in the care of LGBT+ individuals / Revisão sistemática sobre competências de profi ssionais de saúde na assistência ao público LGBT+

This review encompasses both national and international literature about training needs analyses for health care professionals working with the LGBT+ population. The descriptors "health professionals", "health providers", "LGBT","training needs analysis" were searched in the following databases: Academic OneFile, Academic Search Complete, Medical Literature Analysis and Retrieval System Online, Virtual Health Library, Cumulative Index to Nursing and AlliedHealth Literature, Complementary Index, Business Source Complete, Education Source, Gender Studies, National Center for Biotechnology Information, Scientifi c Electronic Library Online, SportDiscus, and PsycINFO. Inclusion criteria were:empirical articles, published from 2007 to 2017, in indexed and peer-reviewed journals, available in full text, in Portuguese or English versions. Of the 17 included articles, most were published in North American countries. There is a need for training and formal protocols within institutions. Training had positive results. No instruments about the competencies for health care professionals working with the LGBT+ population were identifi ed. Further studies should be conducted in the fi eld of health care professionals training to provide satisfactory health care services to this public.

Revisar a literatura nacional e internacional quanto à análise de necessidades de treinamento para profi ssionais de saúde no atendimento ao público LGBT+. Os descritores "health professionals", "health providers", "LGBT", "training needs analysis" foram pesquisados nas seguintes bases de dados: Academic OneFile, Academic Search Complete, Medical Literature Analysis and Retrieval System Online, Biblioteca Virtual em Saúde, Cumulative Index to Nursing and Allied Health Literature, Complementary Index, Business Source Complete, Education Source, Gender Studies, National Center for Biotechnology Information, Scientific Electronic Library Online, SportDiscus e PsycINFO. Os critérios de inclusão utilizados foram artigos empíricos, publicados entre 2007-2017, em revistas indexadas e avaliação por pares, textos na íntegra, em português ou inglês. Nos 17 artigos selecionados prevaleceram publicações norte-americanas; há uma carência de capacitações e de protocolos científicos em instituições; os treinamentos realizados tiveram resultados positivos; e nenhum instrumento sobre competências de profissionais de saúde na assistência ao público LGBT+ foi identificado. Pesquisas devem ser realizadas no domínio de capacitações dos profissionais de saúde para prestar assistência satisfatória a esse público.

Sexuality education for health professionals: A literature review / Ações educacionais em sexualidade para profissionais de saúde: uma revisão de literatura

Abstract The objective of this study was to review the scientific literature addressing educational actions related to the training of health professionals in sexuality between 2003 and 2013. The results obtained show that college seniors, recent college graduates, or those working in hospitals and other health care facilities are not adequately prepared to meet patients' needs regarding sexuality. Several studies have shown improvement in the health practitioners' ability to deal with patients' sexuality issues after participating in training programs in sexuality, regardless of the course load. According to the articles reviewed, in order to improve the performance and comfort level of health practitioners to deal with patients' sexuality concerns, investments in educational actions are necessary.

Resumo Este estudo teve por objetivo analisar a literatura científica relacionada a ações educacionais para capacitação de profissionais de saúde na área da sexualidade, no período entre 2003 e 2013. Os resultados mostram que tanto estudantes do último ano quanto profissionais recém-formados ou vinculados a uma organização hospitalar há mais tempo estão despreparados para atender às atuais necessidades dos pacientes em relação a questões de sexualidade. Pesquisas indicam melhoria da capacidade dos profissionais em lidar com questões de sexualidade dos pacientes após participarem de treinamentos nessa temática, independentemente da carga horária do curso. Para melhorar o desempenho e conforto dos profissionais de saúde em relação a questões de sexualidade dos pacientes, fazem-se necessários investimentos em ações educacionais nessa área, como mostram as pesquisas analisadas.

The sexual life of women with breast cancer: meanings attributed to the diagnosis and its impact on sexuality / A vida sexual da mulher com câncer de mama: significados atribuídos ao diagnóstico e suas repercussões na sexualidade

Breast cancer is the main neoplasm which affects women. It brings emotional problems in addition to physical and social problems due to affecting a bodily symbol of femininity. The aim of this study was to investigate the sexual life of women with breast cancer in the first year after the surgical procedure, seeking the meanings they attributed to the diagnosis and its repercussions on sexuality. Ten women who participated in a rehabilitation program were interviewed. In addition to the face to face interview their medical record were analyzed. Two categories emerged from the thematic analysis highlighting the negative and the positive impacts of this disease on the sexual life. This variety of meanings encountered shows that there no single pattern of sexual life after breast cancer. The way each woman reacts to the disease makes the way she experiences her sexuality unique. It follows that issues of sexuality must be incorporated in interventions offered in the context of care for these women.(AU)

O câncer de mama é a principal neoplasia que acomete as mulheres. Por afetar um símbolo corpóreo da feminilidade, produz alterações emocionais, que se somam às físicas e sociais. Este estudo teve por objetivo investigar a vida sexual da mulher com câncer de mama no primeiro ano após o procedimento cirúrgico, buscando-se demarcar os significados atribuídos ao diagnóstico e suas repercussões na sexualidade. [Do the physical discomforts from breast cancer treatments affect the sexuality of women who underwent mastectomy?]. / Desconfortos físicos decorrentes dos tratamentos do câncer de mama influenciam a sexualidade da mulher mastectomizada?

The objective of this integrative review is to analyze the scientific production addressing the sexuality of women with breast cancer following mastectomy, focused on the effects that the physical discomfort due to cancer treatments have on their sex life. The search included articles published in the period between 2000 and 2009 on the MEDLINE, LILACS and PsycINFO databases, using the following descriptors: mastectomy, breast neoplasms, sexuality, sexual behavior, amputation, psychosexual development, and marital relations. Nine articles were selected, which addressed the effects of the physical discomfort from cancer treatments on the patients' sexuality. The findings revealed that, even when the patient's sex life is intense and fulfilling before the disease, factors such as stress, pain, fatigue, insult to body image, and low self-esteem due to the treatments may alter the sexual functioning of the affected woman. Healthcare professionals must be sensitized in order to welcome and include the topic in policies as well as in preventive, diagnostic, and therapeutic strategies.

Este trabalho é uma revisão integrativa, que objetiva analisar a produção científica dedicada à sexualidade da mulher com câncer de mama após a mastectomia, com foco na interferência dos desconfortos físicos decorrentes dos tratamentos sobre sua vida sexual. O estudo abrangeu trabalhos publicados no período de 2000 a 2009, utilizando as bases MEDLINE, LILACS e PsycINFO, por meio dos descritores mastectomy, breast neoplasms, sexuality, sexual behavior, amputation, psychossexual development, marital relations. Foram selecionados nove artigos, que abordavam as repercussões dos desconfortos físicos provenientes dos tratamentos oncológicos na vivência da sexualidade. Os achados evidenciaram que, mesmo quando existe intensa e satisfatória vida sexual no período prévio à doença, fatores como estresse, dor, fadiga, insulto à imagem corporal e baixa autoestima, decorrentes dos tratamentos, podem desorganizar o funcionamento sexual da mulher acometida. É necessário sensibilizar os profissionais para acolherem o tema em políticas e estratégias preventivas, diagnósticas e terapêuticas.

El estudio incluye trabajos publicados entre 2000 y 2009, utilizando las bases MEDLINE, LILACS y PsycINFO, utilizando los descriptores mastectomy, breast neoplasms, sexuality, sexual behavior, amputation, psychossexual development, marital relations. Fueron seleccionados nueve artículos abordando las repercusiones de la incomodidad física provocada por los tratamientos oncológicos en la experiencia de la sexualidad. Los hallazgos evidenciaron que, incluso existiendo intensa y satisfactoria vida sexual en el período previo a la enfermedad, factores como estrés, dolor, fatiga, insulto a la imagen corporal y baja autoestima derivados del tratamiento, pueden desorganizar el funcionamiento sexual de la mujer afectada. Es necesario sensibilizar a los profesionales para atender el tema en políticas y estrategias preventivas, diagnósticas y terapéuticas.

Mulheres acometidas pelo câncer mamário são confrontadas com a retirada de uma parte do corpo relacionada à sexualidade. Este estudo teve por objetivo investigar o impacto do câncer de mama e da mastectomia na sexualidade da mulher por meio da análise da produção científica publicada no período de 2000 a 2009. O levantamento bibliográfico foi realizado a partir das bases de dados LILACS, MedLine e PsycINFO. Foram selecionados 29 artigos, que constituíram o corpus do estudo. Os achados foram agrupados por meio de análise de conteúdo. Os resultados evidenciaram que, após a retirada da mama, o desempenho sexual é comprometido, com redução da freqüência de intercurso sexual nos primeiros estágios do tratamento e diminuição da excitação, interesse e satisfação sexual.(AU)

Women affected by breast cancer are faced with the removal of a body part related to sexuality. This study aimed to investigate the impact of breast cancer and mastectomy in women's sexuality through the analysis of scientific literature published from 2000 to 2009. The bibliographic survey was carried out in LILACS, MedLine and PsycINFO databases. Twenty nine articles were selected to constitute the corpus of the analysis. The findings were grouped by means of content analysis. The results showed that after the removal of the breast, sexual performance is compromised, with reduced frequency of sexual intercourse in the early stages of treatment and low sexual desire, interest and satisfaction.(AU)
